From the picture it looks like the adaptor has 5 wires. Can i attach this to 1 rear speaker (2 wires ) or do i need to attach it to both left/right sides. Also is the 5th wire remote or a ground cable?

Almost there mate:

2 wires to the Left rear speakers,
2 wires to the Right rear speakers,
1 wire, is the ground cable.
